Your guide to providing the correct information.

If you’re currently living in the UK, you’ll need to complete the Confirmation of Customers Details Form (CNCD). 

Select which circumstance applies to you so you can find the correct information to complete the form and if any evidence is required (photocopies only):

 

You’re employed

Complete all of your employment details in Section 2.  Don’t forget your:  

  • Employer’s PAYE reference number  – you’ll find it on your P60 and sometimes on your payslip.  It will be three numbers followed by a combination of letters and numbers.  Contact your payroll department if you can’t find it.
  • Payroll or branch number – if you have one, you can get this from your payroll department.

You don’t need to return any evidence with your form

You’re self-employed

Complete Section 2 with your Unique Tax Payer Reference (UTR) number.

You can find your Unique Tax Payer Reference (UTR) number in the top right hand corner of your Self Assessment tax return.  This is a ten digit number provided by HMRC.

No evidence is required if you provide your UTR

If you don’t have a UTR, you’ll need to provide evidence of how you’re supporting yourself financially.  Are you:

  • Living from savings; or
  • Supported by someone else.

You’re unemployed

Get your Benefits Office to complete and stamp the “Benefits Office Confirmation” in Section 3 of the form. 

Alternatively, send one of the following pieces of evidence with your completed form:

  • Jobseekers Allowance confirmation (must be dated within the last 3 months);
  • confirmation of other benefits (must be dated within the last year);
  • bank statement showing benefit payments (must be dated within the last 3 months and show your name);
  • Working/Child Tax Credit full award letter confirming your income as zero (must be dated within the last 12 months);
  • confirmation of overseas state support (must be dated within the last 12 months).

We can’t accept evidence of Child Support or Housing Benefit.

When we receive your completed form and evidence, we’ll update your records to show that no repayments are required at this time.

You’re still studying

If you’re in further study we need confirmation that you’re attending in the current academic year.  Don’t send an offer letter from a university or college, as this doesn’t confirm you’ve taken the place on the course and will not be accepted. 

Instead, send one of the following pieces of evidence with your completed form:

  • bank loan agreement covering current academic year for example Career Development Loan;
  • grant letter covering current academic year;
  • letter from the university or college confirming your place on the course (not an offer letter);
  • student card covering current academic year.

Alternatively, if you’re financially supported by someone else, have them fully complete the Third Party Declaration in Section 3 of your form.  To be accepted you’ll also need to send a bank statement which must:

  • be in your name;
  • be dated within the last three months; and
  • show activity with frequent withdrawals being made (ISA accounts will not be accepted).

The purpose of sending your bank statement is to show where money is being drawn from for day to day living.  If it doesn’t show this it won’t be accepted.  Additionally, statements showing money being credited to your account frequently and/or of a sizable amount which are unexplained (and no evidence provided) will also not be accepted.

When we receive your completed form and all the evidence we need, we’ll update your records to show that no repayments are required at this time.

You’re living from savings

If you’re living from savings, you’ll need to return your completed form with copies of your bank statements.  To be accepted, your bank statement must:

  • be in your name;
  • be dated within the last three months; and
  • show activity with frequent withdrawals being made (ISA accounts will not be accepted).

The purpose of sending your bank statement is to show where money is being drawn from for day to day living.  If it doesn’t show this it won’t be accepted.  Additionally, statements showing money being credited to your account frequently and/or of a sizable amount which are unexplained (and no evidence provided) will also not be accepted.

You’re volunteering

If you’re currently doing volunteer work, you’ll need to return your completed form witha letter from the organisation confirming the support they provide you (must be dated within the last 12 months).

You’re supported by someone else

If you’re financially supported by someone else, have them fully complete the “Third Party Declaration” in Section 3 of your form.  You’ll also need to send a bank statement showing the funds you’re living from.  To be accepted, your bank statement must:

  • be in your name;
  • be dated within the last three months; and
  • show activity with frequent withdrawals being made (ISA accounts will not be accepted).
